Eligibility

To register with this surgery, you must live inside our practice boundary.

The edges of the practice boundary are:

  • the North Circular, to the north
  • Dyson's Road, Willoughby Lane or Shelbourne Road, to the east
  • The Roundway or Bruce Grove or Dowsett Road, to the south
  • Wolves Lane, to the west

How to register

To register at the practice please complete the registration form below and attach documents for proof of ID.

  • proof of address
  • proof of immigration status
  • an NHS number

If you cannot email the documents, you can drop them off at the practice reception.
